• A copy of the receipt or proof of purchase for the food you ate

• Details of anybody else who fell ill, for instance, did you go to a wedding where several guests became ill?
• A timeline of what else you ate in the days before getting sick
• Details of any pre-existing health issues

• Any contact you have had with Public Health England.
